DISCLOSURE REGARDING BACKGROUND INVESTIGATION

DRVU (“the Company”) may obtain information about you for employment purposes (including independent contractor or driver engagement) from a third-party consumer reporting agency. Thus, you may be the subject of a “consumer report” and/or an “investigative consumer report” as defined by the Fair Credit Reporting Act (FCRA). These reports may include information about your criminal history, motor vehicle records (“driving records”), verification of social security number, verification of your address history, and other information about your background, character, general reputation, personal characteristics, and mode of living.

The consumer report(s) and/or investigative consumer report(s) may be obtained in connection with your application for engagement or at any time during your association with the Company.

You have the right, upon written request made within a reasonable time, to request whether a consumer report has been run and to request disclosure of the nature and scope of any investigative consumer report. The scope of this disclosure is not limited to the present and may include future updates.

AUTHORIZATION

I hereby authorize DRVU to obtain consumer reports and/or investigative consumer reports about me at any time after receipt of this authorization and throughout the duration of my engagement as a rideshare driver, to the extent permitted by law.

I authorize all persons, employers, schools, companies, corporations, law enforcement agencies, and courts to release information concerning my background, including but not limited to driving records, criminal history, and employment verification to the extent permitted by law.

Acknowledgment and Authorization

I have read and understand this Disclosure and Authorization form. By signing below, I hereby authorize the obtaining of consumer reports and/or investigative consumer reports by the Company at any time during the application process and, if applicable, throughout my engagement.
